This full-time opportunity supports active-duty military members, retirees, and their families in partnership with Ft. This role focuses heavily on patient education, disease prevention, chronic condition management, and promoting healthier lifestyles through proactive outreach and engagement.<br><br><strong>Essential Responsibilities<br><br></strong><ul><li> Promote healthy lifestyle habits and wellness initiatives for enrolled patients, with a focus on individuals with chronic conditions or increased health risk factors </li><li> Conduct retrospective reviews of patient medical records to identify individuals with specific diseases, diagnoses, or conditions </li><li> Refer and connect patients with health promotion and disease management programs designed to support improved health outcomes </li><li> Document patient interactions and program participation within ALTHA </li><li> Educate providers and clinic staff on available health promotion programs and referral processes </li><li> Support patient engagement through education, coaching, and preventive care initiatives <br><br></li></ul><strong> Qualifications <br><br></strong><ul><li> U.S. Citizenship required </li><li> Bachelor of Science in Nursing (BSN) from an accredited nursing program and/or graduate of an accredited National League of Nursing (NLN) school </li><li> Current, unrestricted Registered Nurse (RN) license in any U.S. state, territory, or the District of Columbia </li><li> Minimum of six (6) years of nursing experience preferred </li><li> At least two (2) years of recent experience in public health nursing, disease management, health education, or related specialty areas strongly preferred </li><li> Certification in health education or public health is highly encouraged within the first two years of employment </li><li> Current Basic Life Support (BLS) certification required <br><br></li></ul><strong> Join a team dedicated to improving the health and well-being of those who serve our country while enjoying a rewarding career in a collaborative outpatient environment. <br><br></strong><strong>For More Information<br><br></strong>Kara Smiley<br><br>Kara_Smiley@SpectrumHealth.com<br><br>314-744-4151<br><br><strong>Company Overview<br><br></strong>At Spectrum, we utilize over thirty-five years of experience providing optimal solutions for federal agencies that are both innovative and cost-effective.
