After Care Coordinator

We are seeking a compassionate, organized, and recovery-focused Aftercare Coordinator to join our multidisciplinary treatment team. This vital role helps individuals successfully transition from residential treatment into the next phase of their recovery by connecting them with community resources, treatment services, housing, healthcare, and recovery supports.<br><br>The Aftercare Coordinator serves as a key advocate for individuals served, helping them navigate complex systems and access the resources needed to achieve long-term recovery, stability, and independence.<br><br><strong>Essential Responsibilities<br><br></strong><ul><li>Initiate, coordinate, and facilitate referrals to appropriate community-based services and supports based on assessed needs.</li><li>Provide information, referrals, and follow-up services as outlined in individualized treatment plans.</li><li>Maintain working knowledge of ASAM criteria and available internal and external treatment resources.</li><li>Collaborate with treatment team members, family members, referral sources, and community providers to ensure successful transitions to the next level of care.</li><li>Research and communicate available treatment, housing, healthcare, employment, entitlement, and recovery support resources.</li><li>Complete transition plans and all required documentation in a timely and accurate manner.</li><li>Gather information and complete EIM/ESM enrollments.</li><li>Coordinate follow-up appointments with medical, behavioral health, and recovery support providers.</li><li>Collaborate with program leadership and admissions staff to facilitate internal referrals when appropriate.</li><li>Advocate for individuals served and assist in removing barriers to continued treatment and recovery.<br><br></li></ul><strong>Qualifications<br><br></strong><ul><li>High School Diploma or GED (required)</li><li>Minimum one year of experience working in substance use disorder treatment, recovery support services, case management, or a related human services field (required)</li><li>Strong organizational, communication, and problem-solving skills</li><li>Proficiency with email, internet applications, electronic health records, and Microsoft Office products</li><li>Familiarity with community referral resources</li><li>Experience working in residential substance use disorder treatment settings.<br><br></li></ul><strong>HOURS: Monday-Friday, 9am-5:30pm<br><br></strong><strong>PAY: $25/hour<br><br></strong><strong>How To Apply<br><br></strong>If you are interested in this opportunity, please click “Apply for Job” below or visit our website at www.bhnworks.org and click on “Browse All Jobs” to apply!
